Before your consultation please complete the patient forms

During your first visit, our team will take a diagnostic record of your smile. This includes 3D x-rays, scanning, intra oral, and extra oral pictures. Dr. Cruz will discuss your treatment options and treatment timelines for your particular case.
Our financial coordinator will discuss options to make your treatment as affordable as possible.

Insurance & Financial Options

Insurances

We are currently accepting Delta Dental and MetLife plans as well as the majority of PPO insurances. We are happy to submit your insurance for you! If your plan doesn’t have orthodontic coverage, we will find accommodations to make treatment affordable to you.

No Insurance Patients: In-House Financing

We offer interest free in- house financing that comes directly from a major credit card, Visa, Master Card, Discovery, American Express.

Our office accepts CareCredit. A dedicated health and wellness credit card that may allow you to pay out of pocket costs over time and patients can use to pay for orthodontic care and reserve their general-purpose credit cards for other expenses.

Pay in Full

We extend a courtesy to patients who choose to pay in full.
